If your medical records have not been verified as received by us, please bring copies with you along with any imaging reports with accompanying imaging CDs.
If you need to cancel or reschedule please notify us 48 hours in advance.
If you are experiencing any signs/symptoms of illness, please call us immediately. Symptoms include but are not limited to: fever, chills, cough, altered or loss of smell/taste, abdominal pain, nausea, vomiting, diarrhea.
Once you park your vehicle in the parking lot, please call us at 972-833-PAIN (7246) so we can start the check in process over the phone. We will instruct you on when to come in.
A facemask that covers your nose and mouth is required. Hand sanitizer is available in the clinic for your use.
We will perform a temperature check and ask screening questions for COVID.
