New Patient Instructions

Please complete your new patient paperwork prior to your appointment and bring this paperwork with you.
  • Please remember to bring a valid ID and your insurance card(s).
  • Please arrive 30 minutes prior to your schedule appointment time.
  • If your medical records have not been verified as received by us, please bring copies with you along with any imaging reports with accompanying imaging CDs.
  • If you need to cancel or reschedule please notify us 48 hours in advance.
  • If you are experiencing any signs/symptoms of illness, please call us immediately. Symptoms include but are not limited to: fever, chills, cough, altered or loss of smell/taste, abdominal pain, nausea, vomiting, diarrhea.
  • Once you park your vehicle in the parking lot, please call us at 972-833-PAIN (7246) so we can start the check in process over the phone. We will instruct you on when to come in.
  • A facemask that covers your nose and mouth is required. Hand sanitizer is available in the clinic for your use.
  • We will perform a temperature check and ask screening questions for COVID.
